Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our commitment to your privacy
HYBRD Health PLLC is required by law to maintain the privacy of your protected health information (“PHI”), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. PHI is information that identifies you and relates to your health, the care we provide, or payment for that care.
How we may use and disclose your health information
- Treatment. To provide and coordinate your care. For example, we share your lab orders and results with the laboratories that perform your testing, and your physician reviews your diagnostics to build your protocol.
- Payment. To bill and collect payment for your membership and services, such as processing your payment through our payment processor. We do not bill insurance.
- Health care operations. To run the practice: quality review, secure record keeping through our technology vendors under business associate agreements, appointment reminders, and communicating with you about your care through the LACTIC app, email, or phone.
- People involved in your care. With your agreement, we may share relevant information with a family member or other person you involve in your care.
- As required or permitted by law. Including public health reporting, abuse or neglect reporting, health oversight activities, judicial and administrative proceedings, law enforcement requests that meet legal requirements, to avert a serious threat to health or safety, workers’ compensation, and specialized government functions.
Uses and disclosures other than these require your written authorization, including most uses of psychotherapy notes, marketing, and any sale of your information (which we do not do). You may revoke an authorization at any time in writing, except to the extent we have already relied on it.
Your rights
- See and get a copy of your records. You may inspect and receive a copy of your health information, electronically or on paper. Much of it is available to you at any time in the LACTIC app.
- Ask us to correct your records. If you believe information is wrong or incomplete, you may request an amendment. We may deny the request in limited circumstances and will explain why in writing.
- Get a list of certain disclosures. You may request an accounting of disclosures we made of your PHI, other than for treatment, payment, operations, and certain other exceptions, for the six years before your request.
- Request restrictions. You may ask us to limit how we use or share your information. We are not required to agree, except that we must honor a request not to share information with a health plan when you paid for the item or service in full out of pocket. Because HYBRD Health does not bill insurance, this protection applies broadly to your care with us.
- Request confidential communications. You may ask us to contact you in a specific way or at a specific location, and we will accommodate reasonable requests.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Be notified of a breach. We will notify you if a breach occurs that compromises the privacy or security of your unsecured PHI.
To exercise any of these rights, contact us using the information below. We will respond within the timeframes required by law.
Changes to this notice
We may change this notice and make the new notice apply to information we already hold. The current notice is always posted on this page with its effective date, and a copy is available at the clinic on request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at the contact below, or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
