Hyperbaric Oxygen Therapy: What the Evidence Supports

Hyperbaric oxygen therapy has moved from dive medicine into mainstream longevity conversations, and the marketing has moved faster than the evidence. Here is an honest tour of what HBOT is, where the science is solid, and where it is still early.

What it is

HBOT means breathing 100 percent oxygen inside a chamber pressurized above sea level pressure, typically 1.5 to 3 atmospheres. Under pressure, oxygen dissolves directly into plasma at many times the normal amount, reaching tissue that damaged or constricted blood vessels serve poorly. Sessions run 60 to 120 minutes, and protocols usually involve a series of them.

Where the evidence is strong

HBOT has more than a dozen FDA recognized indications built on decades of use: decompression sickness, carbon monoxide poisoning, non healing diabetic foot wounds, radiation tissue injury after cancer treatment, severe infections, compromised skin grafts, and sudden hearing loss among them. The unifying theme is oxygen starved tissue that needs help healing. For these, HBOT is standard medicine, not alternative medicine.

Where it is promising but early

The interesting research frontier is elsewhere:

  • Cognition and brain injury: trials in post concussion syndrome and stroke recovery show improvements, though study quality varies and sham controlled trials are genuinely difficult to run.
  • Aging biology: a widely cited Israeli trial protocol reported changes in markers like telomere length and senescent cell counts after 60 sessions. Provocative, but small, and marker changes are not proven lifespan or healthspan outcomes.
  • Athletic recovery: plausible mechanism, thin trial evidence so far.
  • Long COVID and inflammatory conditions: early trials show some symptom improvements; larger replication is needed.

Honest framing: mechanisms are real, early data is intriguing, and the confident claims you see in wellness marketing outrun the published evidence.

Practical considerations

HBOT is generally safe when properly supervised. The common side effect is ear barotrauma, the same pressure discomfort as a plane descent; serious complications like oxygen toxicity seizures are rare at standard protocols. Soft shell home chambers operating at low pressure are a different product than the clinical hard chambers used in the research, and much of the evidence does not transfer to them. Cost matters too: wellness protocols often prescribe 40 sessions, which adds up quickly for uncertain benefit.

The takeaway

For its established medical indications, HBOT is proven and sometimes limb saving. As a longevity intervention, it is a legitimate research frontier that has not yet earned its marketing. If you are considering it, know which of those two categories your goal falls into.
