Creatine: What 30 Years of Research Actually Shows

Creatine is the most studied supplement in sports science, with hundreds of trials across more than three decades. It is also one of the most misunderstood. Here is what the evidence actually shows and why it is the first supplement in my own stack.

What creatine is

Creatine is a compound your body makes and stores mostly in muscle, where it regenerates ATP, the immediate energy currency for short intense efforts. You also get some from meat and fish. Supplementing raises muscle creatine stores about 20 to 40 percent, which is the entire mechanism: more rapid energy regeneration for hard efforts, rep after rep.

What the evidence supports

  • Strength and power: meta analyses consistently show greater strength and lean mass gains when creatine is added to resistance training, modest per month but meaningful across a year.
  • Older adults: creatine plus resistance training outperforms training alone for preserving muscle and function in aging, which is why I consider it far more than an athlete supplement.
  • Brain: the newer and genuinely interesting frontier. The brain uses creatine too, and trials suggest cognitive benefits under stress conditions like sleep deprivation, with ongoing research in depression and concussion recovery. Early but promising.
  • Safety: in healthy people, decades of studies show no evidence of kidney damage at standard doses. It is among the best safety profiles in the entire supplement industry.

The myths

It is not a steroid. It is closer to a fuel storage upgrade than a hormone.

The scale change is water, at first. Muscle creatine pulls water into the cell, so expect a pound or two early. That is intracellular hydration, not fat, and the later gains are actual tissue.

It does not cause hair loss. This myth rests on one small study measuring a hormone, not hair. Larger reviews have not supported it.

Loading is optional. A loading week saturates faster, but a steady daily dose gets you to the same place in about a month.

How to take it

Creatine monohydrate, 5 to 10 grams daily, any time of day, with or without food. I recommend 10 grams for larger individuals and anyone interested in the cognitive angle. Consistency matters more than timing, because the goal is saturated stores, not an acute effect. Skip the exotic forms; monohydrate is the version with the evidence and it costs the least.

The takeaway

If you train and you were only going to take one performance supplement, this is the one. Cheap, safe, boring, and effective is exactly what you want.
