How to Optimize Gut Health
By John Wenhold, D.O.
Your gut does more than just digest food. It plays a key role in your overall health. From supporting your immune system to influencing mood, energy and even skin health, a balanced gut microbiome is essential for feeling and functioning your best.
A previous blog post titled "What is the gut microbiome" discussed the gut microbiome in detail and why maintaining a healthy gut microbiome is important. The post explained how by feeding your gut microbiome with prebiotic foods (mainly fiber), your gut microbiota will produce short chain fatty acids (SCFAs). These SCFAs go on to benefit your body in a multitude of ways.
While nourishing your gut with the right foods and fibers is the foundation for producing short-chain fatty acids and supporting whole-body health, diet isn't the only lever we can pull. In addition to nutrition, certain supplements and even targeted peptides can further optimize gut health. These tools can help reinforce the intestinal barrier, modulate inflammation, and encourage a balanced microbiome — providing another layer of support alongside a nutrient-rich diet.
Protocol to Optimize Gut Health: Feed, Protect & Heal
We take a 3 step approach to optimize gut health.
#1 Feed the Gut
The first step in supporting gut health is to feed your microbiome. A well-fed gut thrives on the right fuel, which comes from both food and targeted supplementation. By providing the right substrates, we can encourage the growth of beneficial bacteria and promote the production of compounds — like short-chain fatty acids — that play a central role in whole-body health.
Prebiotic Foods & Fiber
Prebiotics are nondigestible fibers that serve as food for your beneficial gut bacteria. When these fibers are fermented, your microbiome produces short-chain fatty acids (SCFAs), particularly butyrate, which help maintain a strong gut barrier, regulate immune function, and reduce inflammation. Incorporating foods rich in prebiotics creates the foundation for a healthy microbiome. These foods not only help beneficial bacteria flourish but also support long-term resilience of the gut ecosystem.
AG1 (Greens Supplement)
Even with a balanced diet, it can be difficult to consistently get all of the micronutrients, adaptogens, and plant compounds that support gut and overall health. AG1 is a comprehensive greens supplement that provides vitamins, minerals, probiotics, and phytonutrients in a convenient daily serving. The probiotics reinforce microbial diversity, while digestive enzymes and adaptogens help optimize nutrient absorption and stress resilience. This daily "nutrient insurance policy" helps fill in dietary gaps and provides consistent support for both gut and systemic health.
NuButyrate (Butyrate Supplementation)
Every chronic disease has one thing in common: dysregulated inflammatory gene expression. At the center of this dysfunction sits NF-κB, a transcription factor that controls more than 500 inflammatory genes. When NF-κB is overactive, the result can be a wide range of inflammatory conditions — including autoimmune disease, gut dysfunction, chronic pain, and skin conditions.
Butyrate, a short-chain fatty acid naturally produced by beneficial gut bacteria, directly inhibits NF-κB through multiple mechanisms. Optimal butyrate levels have been shown to:
- Reduce intestinal permeability ("leaky gut")
- Decrease inflammatory markers
- Improve insulin sensitivity
- Enhance regulatory T cell function
- Support mucosal barrier integrity
NuButyrate is a next-generation form of butyrate supplementation designed to deliver this powerful molecule exactly where it's needed most — the colon — where NF-κB modulation and inflammatory gene suppression occur. By supporting healthy butyrate levels, NuButyrate strengthens the gut barrier, reduces inflammation, and promotes immune balance.
#2 Protect the Gut
Once the gut is properly nourished, the next step is to protect it. A strong intestinal barrier is critical for keeping inflammation in check and preventing harmful substances from entering circulation. When this barrier is disrupted, chronic inflammation can take root and drive the development of multiple diseases.
Larazotide: Repairing Tight Junctions
The cells lining your gut are held together by structures called tight junctions, which act like gatekeepers — allowing nutrients to pass through while keeping harmful substances out. This barrier is regulated by a protein called zonulin. When zonulin levels rise, these tight junctions loosen, increasing gut permeability (often referred to as "leaky gut").
- What drives zonulin up? Dysbiosis (imbalanced gut bacteria), lipopolysaccharides (LPS), and certain strains like proteobacteria can all increase zonulin.
- Why is this a problem? Elevated zonulin doesn't just affect the gut — it can also be released into circulation, where many tissues have zonulin receptors. This systemic effect contributes to widespread inflammation.
Larazotide is a unique peptide that antagonizes zonulin, keeping those tight junctions sealed. By preserving gut integrity, it prevents LPS and other inflammatory triggers from leaking into the bloodstream — reducing the systemic inflammation that underlies many chronic conditions.
Key benefits of Larazotide include:
- Restoring intestinal barrier integrity
- Preventing the entry of LPS into systemic circulation
- Reducing inappropriate immune activation and inflammation
- Offering therapeutic potential in any chronic inflammatory disease tied to gut permeability
In short, Larazotide acts like a guardian for the gut barrier, ensuring that the protective walls stay strong and inflammation stays low.
#3 Heal the Gut
After feeding the gut and protecting its barrier, the final step is to heal. This phase focuses on repairing existing damage and calming ongoing inflammation to restore optimal gut function. Here, Guttides — a combination of the peptides BPC-157 and KPV — play a powerful role.
BPC-157: The Repair Agent
BPC-157 supports the physical healing of the gut lining. It enhances mucus secretion, promotes blood flow to injured tissues, and accelerates the rebuilding of barrier integrity. Think of it as the "construction crew," repairing microscopic damage and reinforcing the gut wall so it can function effectively again.
KPV: The Inflammation Modulator
KPV targets inflammatory cells within the intestine. It works by blocking TNF-alpha (a key inflammatory cytokine) and inhibiting NF-κB activation, a central driver of chronic inflammation. By calming this inflammatory cascade, KPV helps create an environment where true healing can occur.
Why Healing Matters Beyond the Gut
The gut isn't an isolated system — healing it can have far-reaching effects on metabolism, immunity, and even aesthetics:
- Metabolic Resistance Breakthrough: Poor gut health often fuels an inflammatory state that makes weight loss difficult. By reducing inflammation and restoring gut balance, Guttides can improve metabolic resilience.
- Autoimmune Game Changer: Addressing gut permeability and inflammation at the source helps modulate immune responses and may support long-term remission strategies.
- Aesthetic Medicine Secret: Gut health directly impacts skin health. By reducing systemic inflammation, Guttides can improve healing after cosmetic procedures, calm inflammatory skin conditions, and enhance overall appearance.
- Food Sensitivity Resolution: When combined with Larazotide, Guttides address both sides of intestinal permeability — helping reduce reactions to trigger foods while supporting barrier restoration.
Protocol: Guttides 500 mcg twice daily.
In essence, Guttides bring together the repairing power of BPC-157 and the inflammation-calming action of KPV, making them a cornerstone of gut healing strategies.
The Bottom Line
Gut health is not about a single food, supplement, or peptide — it's about a layered, strategic approach. By first feeding the microbiome with prebiotic fibers, targeted nutrition like AG1, and butyrate support, we provide the raw materials the gut needs to thrive. Next, we protect the intestinal barrier with Larazotide, ensuring tight junctions remain sealed and preventing the constant immune activation that drives chronic inflammation. Finally, we heal with Guttides (BPC-157 and KPV), repairing existing damage, calming inflammation at its source, and creating the conditions for long-term resilience.
Together, these three steps — Feed, Protect, Heal — form a comprehensive gut protocol designed not just to improve digestion, but to influence every area of health: metabolism, immunity, inflammation, skin, and beyond. When the gut is balanced and strong, the rest of the body can follow.
